My Home Care provides Skilled Nursing services to help patients heal at home and regain their independence. Our goal is to provide personalized, expert care to each of our patients to promote their health, safety, and overall well-being.

Our licensed Registered Nurses are trained to provide the personalized, compassionate care to our patients at their home. Our nurses work with patient's physician to administer care unique to the patient's needs. Our nurses will visit the patient's home to assess their needs and develop a plan of care according to the patient's needs. Our Skilled Nursing services include:

Physical Therapy

My Home Care provides Physical Therapy (PT) services to improve the quality of patients' lives by increasing motion and mobility, reducing/eliminating pain, and promoting the prevention of further injury.

Our experienced and licensed PTs will work one-on-one with the patient in the patient's environment; making services completely relevant to the patient's unique needs and goals. The program aims to help improve health and help patients return to a normal lifestyle. Therapy plan may include:

• Mobility Rehabilitation
• Developing a Home Exercise Program for Patient
• Gait Rehabilitation
• Training with Body Mechanics
• Self Care Skills Training
• Physical Therapy for Geriatric Patients

Occupational Therapy

My Home Care's Occupational Therapy focuses on helping you to restore the functions of daily living. Our therapists are highly skilled and compassionate; providing emotional support to our patients and their family members, which is an integral part of the patient's recovery process.

Our OT practitioners provide skilled services to evaluate and address the needs of our patients in relation to their desired occupations so that they can live safely and productively in their home environment. In addition to training to increase independence in dressing, bathing, combing your hair, and other personal care activities in the home, our OT services may also include evaluation of safety in the home and recommendations for improvements, medication management, wheelchair seating and positioning, pain management, falls prevention, and management of chronic diseases.

Home Health Aide

Families know when they are in need of companion care, memory care, transitional care, respite care, elderly care, or other forms of essential personal home care services they can count on My Home Care.
